Since 2002, the Connections Program has provided services to over 132 youth under the legal jurisdiction of the Hennepin County permanency system. Program services address youths' developmental needs with staff working closely with permanent placement providers, social workers, therapists and educators. Specific services include:
Increasing communication between permanent placement providers, schools, Hennepin County and other community resources.
Facilitating school enrollment, attendance, academic progress and planning.
Developing and monitoring Independent Living Plans for each youth.
Providing learning experiences through ventures with other community-based agencies and businesses.
Individual and group Independent Living Skills training.
Promoting and supporting recreational, athletic and creative activities for youth.
Assisting youth with obtaining and sustaining gainful employment.
Assisting youth with locating, obtaining and maintaining housing.
Providing assistance for youth who are pursuing post-secondary education.
